INTRODUCTION
The
student industrial work experience scheme (SIWES) is a skills training program.
This program is been conducted by the industrial training fund (ITF)
established under decree No. 44 of 1971 stale as SIWES and harmonized the progromme
as scheme in 1973. The scheme is for student of science related course science
and technology including environmental studies.
1.1 DEFINITION
OF SIWES
SIWES
is a skill program from which it forms part of approved minimum academic
standards in the various courses for all national and international tertiary
institution. The program is compulsory for all students in science and
technology, it is designed to help the student to acquire practical knowledge
in their area of specification so as to not be limited only in the theoretical
aspect being learnt in the class.
1.2 PURPOSE
OF SIWES
1.
The student industrial work experience
scheme (SIWES) is a skill training program design to expose and prepared the
student of universities, polytechnic, college of education and technology to
real life practice which helps to bridge the gab between theoretical and
practical knowledge.
2.
The scheme provide student opportunities
of being familiarized expose to the use of machineries/equipment which are
usually not available in the education institution.
3.
The scheme expose student to work method
and prepare them in safe guarding the work areas and some other works in the
organization industrial.
1.3 OBJECTIVES
OF SIWES
1.
Prepare students towards the challenges
of work the encounter after graduation.
2.
Make transition from institution to the
world and thus enhance students contact for job placement after graduation.
3.
Enlist and strength employers to the
involvement in entire educational process of preparing the institution graduate
for employment in industry.
4.
Expose the students for the methods and
techniques handling equipment and machinery that may not be available in the
institution of SIWES.
1.4 HISTORICAL BACKGROUND OF YUSUF DANTSOHO
MEMORIAL HOSPITAL T/WADA KADUNA (Y.D.M.H)
The
hospital is based in Kaduna State at the North East of center Tudun Wada Kaduna
Local Government then under the chairmanship of Alhaji Yusuf Dantsoho. It was
commissioned March, 1975 by the effort of his Excellency Governor Abba Kyeri
(Military Administration of Kaduna State) and his Hon. Commissioner of Health
Alhaji Sani Zangon Daura the Hospital was name General Hospital Tudun Wada
popularly known as (Asibitin Dutse). Due to very big mountain neighboring the
hospital as the right side, the hospital has some of the faculties such as
male/female ward. Theatre laboratories maternity, laundry Kettering and
Administration section which are all well equipped and also partner with
NOCACOPLE Maryam Foundation and Government Houses e.t.c.
The
hospital was renamed in 1992 by the Excellency Connell Hamid Ali and Governor
of Kaduna State as Yusuf Dantsoho Memorial Hospital Tudun Wada Kaduna after the
death of the former Chairman of Kaduna Local Government due to regard and
remembrance of his effort and serving the Local Government during his life time
in Kaduna State.
The
hospital was setup to provide medical services along with some other hospital
in the old North Center. Some Hospitals in Nigeria are:

AND STATISTICS UNIT
The
Records and Statistics unit is the Fourth Department in the Hospital which
deals within and out patients appointment fixing e.t.c.
They
are group into various Departments such as:

DEPARTMENT (0 – 12 YEARS)
Pediatric
Department refers to the Department which in concerned with Children whose age
is from 0 – 12 years the Patients were expected to be in the Hospital as early
as possible in order to see the Doctor.

SORTING
OF CARD AND FOLDERS: Sorting is also to arrange card or file
also known as Hand filer this is the arrangement of card being done before the
card can be file into shelve.
CARDS:
Also called OPD Cards the out patient Department Cards is a plano Card which
will be giving by the Card to the Patent before the doctor will attend to the
patient.
FOLDERS:
Is a long file which is used for keeping Document Such as files in patient
notes, lambrequin form e.t.c in order to avoid misplacement of some document.

FILING
OF CARDS FOLDERS IN SHELVES
This
is done immediately after storing which is the arrangement of cards folders in
the shelve accordingly number used to retrieved card from the shelves also
called First number middle and last number.

REGISTRATION
OF PATIENTS
This
is when the patients is using the Hospital for the first term it simply means
the collections term of patient data or information (Bio dates) unit number
such as surname, first name, place of origin, occupation, religion, Sex, Tribe,
Home Address, State, Name of next of Kin and Telephone Number e.t.c.

These are large
register which consist of date of patient who admitted and discharged in the
hospital and it contains the name of the patients series numbers, unit number,
diagnosis e.t.c
Out
Patient Register: These are large register which contain
the data of patient who only come to see the doctor with out admitting in the
hospital.

ENTERING
OF STATISTICS (DAILY OUT PATIENT REGISTER)
Daily
statistics i.e this is the daily patient attendant that come to hospital. This
is the being calculate according to their age, range and sex. The age is from 0
– 28days, 29days – 11Month,12Months – 59 Month, 5 – 9yrs, 10 – 12yrs.

3.2 ENTERING
OF STATISTICS IN PEDIATRIC SECTION
This
is done as early as possible in order to file the cards and folders in the
shelve and card.
SHELVES:
This is a place which is meant for filling, retrieving and keeping of vital
document for easy retrieving.
ENTERING OF OLD CASE:
After the retrieving of card from the shelves the bio dates of the patients
will be enter in the register column that have folder card in the hospital.
ENTERING OF NEW CASE:
This is always done when the patient is using the hospital for the first time
after the purchase of the folder, the bio datas of that patients will be enter
in the register, then signer by the medical that attendance for the day.
CONTINUATION SHEET:
This is white sheet meant for entering the diagnosis of patient after,
treatment by the doctors the sheet, have three column, for the surname , first
name and patient number and sub – column date and noles be seen from sketch
below.

GENERAC ATTENDANCE OF PATIENT
This
is an act of getting the number of patient that use the hospital, in daily
bases or weekly, tabular representation showing the number of registered
patients.

AND REGISTRATION

is define as the collection of patient
information that consist of their name, age, address, next of kin and sex
e.t.c.
when
documenting there is what they called patient number which consist of six digit
number that is being assigned to patient.
When
ever they come in contact with the hospital as the first time. These number is
what the mean by the hospital number.

– NATAL (MARRIED WOMEN)
This
is mainly for pregnant women whose ages are from 13 – above they also make use
of the hospital. This is done by examining the patients before birth, during
pregnancy.

When getting information from the
patient it is very important to get correct and accurate biodata’s about the
patients in order to avoid mistake when putting down.

Misfiling it is very important to check
the number i.e the unit number before filling in shelves.

SUMMARY,
CONCLUSION, RECOMMENDATION
5.1 SUMMARY
The practice experience of composing of one section
Paediatric section: This is a section which
consist of children whose age is form 0 – 12yrs old. The activities carry out
are sorting of file, filing statistics entering and retrieving of cards.
5.2
CONCLUSION
At
the end of the training was able to learn, sorting and filing of cards and
folder, filing of monthly statistics and appointment fixing, retrieval of
folder and plan cards from the shelves.
5.3
RECOMMENDATION
I
Recommendation that Federal Government should be provided places for industrial
attachment for student in order to prepare student for labour market. Also ITF
should ensure that every organization accepting any student should monthly
assist the student financial either weekly or monthly to help the student meet
up with financial challenges.

Patient and guardians should encourage
the student on providing necessary things needed by the student during their
training.
b.
The government should endowed to pay
student for their industrial training as soon as possible.
